New treatment options in adrenocortical carcinoma
Abstract:
Adrenocortical carcinoma (ACC) is a rare and aggressive disease with limited therapeutic options and a high rate of recurrence even after complete resection. Until now, mitotane is the only drug approved for the treatment of advanced ACC. Both the ESE-ENSAT guidelines 2018 and the ESMO-EURACAN guidelines 2020 recommend an adjuvant treatment with mitotane in patients who have a high risk of recurrence (e.g. Ki-67>10%). Nevertheless, the recurrence rate is still about 50% even after mitotane treatment. Furthermore, adjuvant chemotherapy and the use of local therapies in ACC have not been established, and the available evidence is extremely limited. Moreover, the role of second-line therapies in advanced ACC is less defined and only few patients experience objective response to the suggested therapies like streptozotocin or gemcitabine and capecitabine. Furthermore, the molecular mechanisms behind the efficacy of streptozotocin are still largely unknown.
The main aim of our studies is to improve treatment for patients with ACC. Therefore, we investigate the efficacy of different treatment options with retrospective and prospective studies as well as cell culture experiments. We contribute also in several clinical trials and take part in some external collaborations in the ENSAT network.
